Delphi-Derived Frameworks for Mental Health System Redesign: Blueprints for System Change in British Columbia

Mental health service integration remains a critical challenge across health care systems, particularly for vulnerable populations such as birthing people and people with concurrent disorders. This presentation synthesizes findings from two complementary Delphi-studies addressing mental health system redesign through consensus building approaches. Both studies employed a modified e-Delphi approach to identify critical elements of integrated care, including multiple domains and indicators of care.  Despite addressing the needs of different populations, both studies converged on several key findings: 1) the importance of incorporating lived/living experience alongside health professionals in system redesign; 2) the need for structural changes to address fragmentation and siloed care; 3) the importance of culturally responsive, low-barrier approaches; and 4) the critical role of policy in creating conditions for meaningful system change. These findings provide blueprints for health care transformation that transcend traditional disciplinary boundaries, emphasizing integration as both a clinical and organizational imperative. Implementation of these consensus-based frameworks offers promising pathways to enhance service accessibility, improve treatment outcomes, and advance health equity across the mental health system.
