An introduction to Synergy Tool implementation and the impact on long-term care

While long-term care (LTC) delivery has been persistently and negatively impacted by ongoing workforce challenges, resident-centred models of care provide an opportunity to enhance LTC provision and improve resident and provider well-being. The Synergy Model, an evidence-based and resident-centred model of care, facilitates safe staffing and workload management in real time by identifying individual resident acuity and dependency needs (the Synergy Tool), and assigning staff based on their capacity to meet these needs.

This presentation will introduce an ongoing project aimed at improving care delivery for LTC residents by bettering LTC workload management through adapting, implementing, and evaluating the Synergy Model through a multi-phase, mixed-methods study.

The Synergy Tool was implemented for a six-month period in select units within two partner care homes in Vancouver, British Columbia. Quantitative evaluation includes analysis of staff and informal caregiver (e.g., family) surveys pre- and post-Synergy Tool implementation, monthly administrative data (e.g., overtime), and Synergy scores. Qualitative evaluation includes analysis of focus group data with LTC leaders/managers, care providers, as well as residents and caregivers to gain richer insight into the implementation of the Synergy Tool and its impacts. Data analysis is ongoing, yielding only preliminary results.
