Sharing the Decision, not the Burden: Toward Integration of Shared Decision-Making in Medication Use for Heart Failure

Heart failure (HF) affects over 800,000 Canadians and is on the rise. HF develops when the heart is not strong enough to move blood around the body, impairs quality of life, and reduces life expectancy. Despite high-quality evidence supporting several medication options to treat HF, use of these medications remains low, with fewer than 1 in 5 receiving optimal medical therapy. The increasing complexity of HF pharmacotherapy has renewed the call for shared decision-making (SDM) and the development of evidence-based decision aids to bridge this large practice gap and improve the integration of patients’ preferences in decision-making. To date, interventions to support SDM in the care of patients with HF care have focused on offering device therapy and palliative care. The development of SDM tools should be informed by patients’ decisional needs, treatment preferences, and values, which are largely unknown for HF medications.

This Work in Progress Seminar will provide an overview of the ongoing, multi-project SHARE-HF program, including early results. The overall aims of SHARE-HF are to develop and evaluate web-based and simplified paper-based patient decision aids to facilitate the integration of shared decision-making in the delivery of medical therapy for patients with HF. Finally, we will contextualize SHARE-HF in the landscape of ongoing implementation efforts in treating HF across North America.
